Yes on Amiga 500. I noticed it on a game Rick Dangerous when game started itself without any joystick or mouse connected to Amiga, or on mortal combat when second player shows without having second joystick connected.

Ok, so this would definitely seem that your fire button is stuck.
Did you check that your joystick ports do not have any bent pins? |

If the problem is in the Amiga and not the joystick itself, then my bet is on a faulty CIA (8520). Swap the two ICs around and see if it changes.
