Cannot understand how input works

I am trying to setup my x-arcade to work under Winuae. I figured out there is an input menu for it, but it seems that on configuration input I can't seem to locate the Left control alt key . Also once I have gone through mapped my buttons, How do I tell Winuae how to use the config 2 keymapping i made. Game ports give me no option for the defualut. There is custom, but I have no idea what that is? Can't find any video to explain it. Can someone please help me out? I have spent a lot of time trying to figure this out. Thanks

Select Custom #<1-6>, then click Remap/Test. The rest should be mostly logical (at least compared to Input panel)
