Make sure you are using a good known AmigaDOS formatted disk to do the checks - preferably an original magazine coverdisk or Workbench disk. Even using the same disk to check all the drives.

Sounds like the disk presence switch on the drive needs cleaning? Check the switch for continuity and that it works.

To clean it, flood/soak it with IPA and repeatedly press the switch.

disk-analyse does not support HFE v3 yet. Instead, I would recommend using the HxC software to convert IPF files to HFE v3 files.
