The firmware may only designed to hook into a 3.1 rom. Maybe better off using 3.1 and just boot load any updated libs you need. This ensures max compatibility.

It's not documented in the flash archives but it was often suggested when troubleshooting flash update problems in the early ppc days when the cards just came out.

With write protect disabled, winuae should generate a new adf file in the emu folder. 'filename_save.adf'
