Joe, I have a spare Simm I could send you. It might help rule out a problem with your memory.

If you can get with Jack-3D this weekend a quick test of the card would be the best thing to do.

Using a Sonnet and a Deneb in DMA mode, attempting to launch WOS stuff from a USB stick attached to the Deneb causes a hard-lock. I'm assuming this is to be expected as the sonnet cannot DMA to the Deneb? Using the Deneb in pio mode and all is well, no lockups which I guess is also expected as the 060 handles the access, is this correct?

Give me a Deneb and I'll test :-p

I have no clue how DMA comes into play here.

With the Deneb in DMA mode the swap jumper would have to be closed on the Mediator, I assume this is done?

Fingers hugely crossed here! My clean setup, still with CSPPC, not Sonnet, seems to finally run stable. I have played Blitzquake PPC for about an hour and Wipeout for about 45 minutes. Games that would have crashed much sooner everytime with my previous setups.

What I did:
Stripped the A4000 to the bare minimum:
- Standard CDROM on IDE 1 (so no IDEfix)
- Only CSPPC, Mediator and Voodoo 3 3000 in top PCI slot
Installed the following:
- AOS 3.9 BB1&2 + all stuff with versions as close as suggested here in the thread.

What did the trick is still a question, since in the mean time, this is my 3rd Voodoo card, the only one with a huge passive cooler. So it may still have been heat.

I'm gonna stress this setup some more and then I'll clone it for a Sonnet setup!

Good work Spudje! Is this with the case off still? I ended up putting a fan on my Voodoo 3 because I was concerned about it getting so hot...

Wasn't there difference between big silver colored heat sink and black colored small heat sink the V3 3000 vs the V3 2000?

Yes, I thought so too, but my first 3000 with black heatsink still reported as 3000. Maybe flashed firmware? Anyway I'm hoping I'm finally on the road to success

I have no clue how DMA comes into play here.
The Deneb is a Zorro III DMA Master and can perform Zorro III DMA read/write to any location within the Zorro III address space.

I have no knowledge of how the Mediator works but the Sonnet's RAM lives presumably in PCI address space? The "Amiga side" can read/write to this RAM through the Zorro III address space via Mediator hardware. But DMA does not work on the 4000 Di.

Here is a post where the Deneb designer states that DMA mode doesn't work on a mediator 4000 (Di) and he doesn't know why
