All's well then.

What is the exact format for Generation 4 54. I don't see anything stated in the bootblock. Is this a format not yet supported ?

According to HxC track analyser, T00.0 has a bad crc and tracks 35,36,37,38,39 has unknown encoding.
Disk is probably in need of proper cleaning and a redump.

If you type "cap" at the AmigaDOS prompt when you boot the disk you can start the Combat Air Patrol demo.
So it's semi-working, Amiga part is OK.

No startup-sequence on disk, read page 11.

Who made that dump? dlfrsilver? I'd bet money that the disc analyser doesn't support triformat rather than he released a bad dump. He's quite thorough

With SCP file ( after conversion from Raw ) :

The Amiga part is Ok.
The ST part (Entombed + Doc) is Ok with Steem.
The PC part is extracted correctly on the HD (after copying the executable archive to another standard PC Floppy and copying to the HD) and the game demo launches ( DosBox-X + 386/486 Prefetch emulation ).

It was not dlfrsilver who did the dump but a member of the AmigaFrance forum (Jay) who found some floppy disks ( Tilt and Generation 4 ).
