"A3000T-040 68040 Support Library" disk

For historical sake (and my own curiosity) does anyone have a working copy of this rare disk from Commodore (photo below)?

The Amiga Online Review Column via cucug.org's review notes: "Also included was a note telling me to install the 68040.library immediately. A 68040 support disk was included with a neat install script that worked fine."

I own one (bought four from eBay), but the disks themselves are so degraded (mold?) that it actually killed one of my external floppy drives. :-\

Anyway, I'd like to get an ADF of this if someone has a copy or its contents archived somewhere?

I'm also looking for a similar A4000T disk (although high-density) that apparently(?) was included with some A4000T systems.
