I went and contacted SOundQuest because, you know, whatever. Response:

Sorry, the master disks were lost over a decade ago in a move.

give it a try, if you have an account overe there

btw. i was playing with MidiQuest 1.03 - since it is only an update, the drivers/template disk is missing.
so I had a look to the AtariST version on planetemu, and the driver disk is usable with the amiga version. it is just a bit troublesome to use it.
files on atari disk comes in compressed archive (arc) but once extracted and put in placed to the amiga version, the files are accepted.
For example, drivers for MT-32 is there to do some experiments.
i've upload the archive with the drivers/templates into the zone: MidiQuest_v1.0-Drivers-Templates.lha

#1) I removed the archive because it is now obsolete - use MQ3 instead.

wow, thanks a lot. there is allways hope for such rare things. many thanks.
just about to install it, will report back

#1) installation is a pain: create a folder on your hardrive, eg. MidiQuest3 and drag&drop install-hd icon into it
also create a folder MidiQuest3/Instruments and assign Instruments: to it - then start install.
#2) on first start it aks about driver list, let it build and save - much better than in version 1 now.
#3) i wasnt able with MidiQuest1 to get any sound from MT32 emulator munt, with MQ3 now it works
#4) working very good with screen-promoting tools like newmode
(eg. using here uae screen 800x600 )
#5) thanks again, mech

Thank you mech!

About version 4, one year ago I've emailed...
- David Brunet ( http://obligement.free.fr/articles/midiquest.php ) but he is not the author, and he don't have the disks.
- SoundQuest but Michael Lambie's answer was "Sorry, but no. We don’t have anything left from the Amiga days which ended a long time ago"
- Sven ( http://members.aon.at/wunger/ ) but he doesn't answered. At the bottom of the page is a Sammlung excel file an there mentioned Midi Quest 4 for Amiga. The webpage name (wunger) reminds me an Amiga enthusiast, Wolfgang Unger. If somebody knows him, it will worth a try.

Yeah I also wrote to Soundquest and got the same reply, less than a year ago. I wrote above that they said they lost the masters in a move. Hmmm..

Thanks for version 3 though, Mech! I think I can use that with my synths, I will give them a try!
