That's weird. There's not much to it, so it should just work.
Have you tried the composite out on the adapter?
Also, what type of Amiga (Model and NTSC or PAL) and what type of TV (svideo device) are you using?
My adapter works great, but it still displays nothing on my TV if my Amiga is outputting PAL as my TV doesn't support PAL timings.
