Alen, do You have microscope or magnyfuing glass? Check Your resistor networks, i am starting to suspect one of these has a bad solder point. I had similar issue with one of my cards, where it was unstable at higher frequencies, also i had lower (7.5Mip) speed. I was thinking, it's because i have used 15ns chips. After Steve told me, it has nothing to do with chip speed i started to investigate, and found out that one resistor in the resistor network (nearest to the RAM) wasn't soldered, fixing that maked this card rock solid, no fails, excellent working @51.8Mhz
also try to install pullups on motherboard and test with other power supply. Kickstart EProm should be 120ns max. 105ns's costs double the price, but have also small fail rate, where 150ns chips are quite often defective
