Found the issue with the menus - I somehow managed to drop the WFLG_NEWLOOKMENUS flag from the window, resulting in OS 2-style menus, but forcing an OS3 palette caused both foreground and background to be black. It's an easy fix, and doesn't affect MagicMenu (which is probably why I didn't spot it myself).
