1) How exactly do you obtain this "secret" extra life on Round 6 (the Wizball-shooty level)?

2) What exactly triggers the Happy Birthday screen that pops up when you start loading the game sometimes? Seeing how Galahad obviously had to examine all the code when making a WHD install, maybe he might know how the birthday code works.
