Sorry no double barrel screenshot but it sounds like the game Damage to me, written around 1995ish from memory. 2 disks. You escape from the laboratory at the start then find a baseball bat and start the massacre.

The only other thing I can think of which is remotely like that would be Ranx/Ubisoft.
