For imaging ADFs the best tool i've found is DIC from the Whdload package.
If the disk can be read without errors and did work as a copy (i.e. isn't copy protected) then it'll work as adf too.

Of course it will not help if the offset is wrong.
Again, last byte of a long is +3, not +4 (offsets are 0, +1, +2, +3, total 4 bytes).
+4 is first byte of next long.

Now I switch to same resolution but DBLPAL. Image shrinks horizontally.
This does not depend on filter settings.
